This demonstration uses the Mad Blanks Create Single Record module to present a simple 10-question quiz. It shows three ways of presenting the response options to the website user (text fields, radio buttons, and drop-down menus). Each question is worth one point. The answer to each question is the same as the question number. The score script for this quiz is as follows: <p>You earned [:total_points_earned_a:] points out of a possible [:total_points_possible_a:].</p>
